Evaluation of Teleconferences

Instructions: The purpose of this evaluation is to improve the teleconference presentations and topics. There are total of 9 multiple choice items and two open-ended questions. Please complete all questions and click the  SUBMIT button at the end of the questionnaire.   Please take a few minutes to complete.  If you do, we will send you a complimentary $25 voucher good toward one future RSA Region V CRP-RCEP training.  Please give us your name with mailing address in the #12. suggestion box below.

To earn CEUs and/or CRCs please make sure you answer the question in box 12, give us your mailing address and the telephone number you used to participate, and let us know your desired credits. Please allow at least one month for receipt of credit.  Please complete and return no later than one week from teleconference for credit.
