Registration for Disability Services

For any student to receive services based on a documented disability, there are two components for registration with Disability Resources and Educational Services (DRES), the disability division for the U of I:

  1. Fill out the DRES application, either online, or on paper; find it at: www.disability.uiuc.edu/application.
  2. Provide documentation from an appropriate medical source---all major categories of disabilities and requirements are listed here.

Graduation Checklist for EOL Ed.M. Students (non-thesis)

1. Apply for graduation before deadline

Master’s students graduate in May, August, October, and December. In order to graduate, you must be on the pending degree list for the term you intend to deposit. You can add your name to the list by submitting the “Application for Degree” form or by using the UI Integrate Self-Service system. Please add your name even if you are unsure about your graduation because you must do that prior to the deadline. You will be unable to graduate for that term if you miss this deadline. Keep in mind that the application does not carry over to the next graduation term. There is no penalty if you need to re-submit the form.

2. Update address information on UI Integrate Self-Service. Your diploma will be mailed to your DIPLOMA address.

3. For students who need evidence of a master’s degree, complete and submit the “Request for Certification of Degree Form”

4. For students interested in obtaining the Type 75 license, please contact Dr. Hackmann at dghack@uiuc.edu.

new Graduate Student Petition Online

Graduate students are now able to complete the petition form, including an extra page for explanations, electronically. Students will be prompted to provide necessary information relevant for each request, which will mean fewer unclear or incomplete petitions. Students will still need to print the PDF form in order to obtain the necessary signatures. However, the Graduate College will be able to view the electronic version, which means saved time in the petition process. Students will also now have the ability to go online to view the status of their request. The link to the student petition form is www.grad.uiuc.edu/petitions/. A link to petition instructions and an FAQ section are also available there.
